Do people have an obligation to help others or should people be responsible for helping themselves?

Margaret Meade contended that the first sign of civilization was a healed human femur.
Horses have this thing that makes them devilishly hard to save when they're injured (hence so many horses euthanized). Their version of shock causes their muscles to basically de-laminate. It's the no-choice version of a herd animal taking one for the team.
While social, we are not a herd animal, and have evolved to make choices about our sacrifices. Sometimes we choose well. We haven't yet evolved to foresee the consequences of our actions/inaction, still waiting for that upgrade.

In general, we are all better off if we help one another.
